Patient Quick Reference Guide
Limiting Exams Per Hour
Only 2 patients per hour will be allowed in order to provide a thorough cleaning between exams.

Ask about our social distance examination- we are able to get a high definition wide angle look without the need to dilate
Limiting the Number of Patients
We will be limiting the number of patients and exams to adhere to CDC guidelines.

Patients should not be accompanied to exam unless necessary for children and elderly.
Before You Arrive
1. Fill out Patient Intake Forms before you arrive.
2. Call to notify arrival from your car.
3. Come alone unless necessary to have help.
4. Bring a personal face mask.
5. Leave unnecessary items at home.
6. Reschedule if not feeling well.
When You Arrive
Please call us when you arrive to the office.
You may need to wait in your car as we clean between appointments. We will utilize a contactless thermometer to take the  temperature of everyone who enters. We will ask anyone with a fever to reschedule.
Personal Protective Equipment & Sanitizers
Doctors and associates will wear masks and and gloves. We ask that patients bring their own facemasks for personal protection. Gloves and Sanitizer will be available. Sneeze guards will be placed in high contact areas.
Cleaning Equipment and Surfaces
All equipment and surfaces will be cleaned before each exam and during pretesting. High contact surfaces such as handles and and countertops will be cleaned and sanitized throughout the day.
Social Distancing
We ask associates and patients to observe 6 feet of social distancing in accordance with CDC recommendations.

Please keep in mind you may need to wait in your car while we adhere to these standards and allow us to disinfect between appointments.
Hand Hygiene
Our associates will wash their hands using soap solutions for 20 seconds after every patient interaction.

Gloves will be worn by all employees. Patients will be asked to wash their hands with soap solution before their examination.

Contact Lens Wear is Safe, however:
• Proper Hand Washing is Essential.
• Disinfect Contact Lenses.
• Discontinue Lens Wear Only if Sick.

Also remember to:
• Disinfect Your Eyeglasses.
• Eyeglasses are Not Proven to Offer Protection.

Symptoms
• Mild to severe respiratory illness with fever, cough, and difficulty breathing
• Symptoms may appear in as few as two days or as long as 14 days


Transmission
• Spreads person to person through small droplets from nose or mouth which are spread when a person with COVID-19 coughs or exhales.
• Can catch the virus by touching objects and surfaces that droplets have landed on and then touching their eyes, nose or mouth.


Steps to take to minimize risk
• Wash hand with soap & water for at least 20 seconds
• Avoid touching your eyes, nose, and mouth with unwashed hands.
• Avoid close contact with people that are sick.
• Stay home when you are sick.
• Cover your cough or sneeze with a tissue, then throw away the tissue in trash.
• Clean and disinfect frequently touched objects and surfaces.
• Ensure employees have ample facilities to wash their hands.
   
PROTECT YOURSELF AND YOUR FAMILY
Covering your mouth when you cough or sneeze with your elbow or a tissue will greatly reduce the number of germs that are able to spread to people and objects around you.

Even if your illness doesn't seem very bad to you, it could be much more serious for someone who catches your germs.

SOCIAL DISTANCING

What Is Social Distancing? Social distancing is a way to keep people from interacting closely or frequently enough to spread an infectious disease. Here in the office we're asking you to not sit next to each other to observe Social Distancing. Everyone reacts differently to stressful situations, please be respectful of others boundaries.
